脓毒症患者血清外泌体对脓毒症大鼠急性肺损伤的影响

摘要:
目的 研究脓毒症患者血清外泌体(Exosomes)对脓毒症大鼠急性肺损伤的影响及其机制。方法 采集脓毒症患者血清样本,采用ExoQuick试剤法从血清样本中分离提纯Exosomes?建立脓毒症大鼠动物模型,并将实验大 鼠随机分为假手术组、脓毒症组、实验组和对照组,每组各8只。对各组大鼠进行动脉血气分析、血清炎症介质检测、支 气管肺泡灌洗液(BALF)白细胞计数、肺组织病理学观察和肺组织湿/干质量(W/D)比值检测。结果 与假手术组比 较,脓毒症组和对照组大鼠PH值、P&O2较假手术组明显降低,而PaCO2则明显升高;实验组大鼠PaO2低于脓毒症组, 而PaCC)2则高于脓毒症组(P<0. 05) o与假手术组比较,脓毒症组和对照组大鼠血清TNF asIL 1[3SIL 6和IL 10的浓 度水平值均明显升高;实验组大鼠血清TNFaJLlpJL6和IL 10的浓度水平值则高于脓毒症组(P<0. 05) o脓毒症 组和对照组大鼠BALF中白细胞数量显著增加,明显高于假手术组;实验组大鼠BALF中白细胞数量则高于脓毒症组 (P<0.05)o脓毒症组和对照组大鼠肺组织W/D比值和病理评分均明显高于假手术组,实验组大鼠W/D比值和病理 评分则高于脓毒症组(P<0. 05)。结论 脓毒症患者血清Exosomes会进一步加重脓毒症大鼠急性肺损伤,其机制可能 与Exosomes会加重肺部炎症反应有关。

Abstract:
Objective To investigate the effect of serum exosomes from patients with sepsis on acute lung injury in sepsis rats and its mechanism. Methods Serum samples of sepsis patients were collected. Exo Quick reagent method was used to isolate and purify exosomes from serum samples. The sepsis rat model was established, and the rats were ran domly divided into sham operation group (exposed and turned, cecum only after caesarean section, without any special treatment after operation) , sepsis group (sepsis rat model without any intervention) ? experimental group (serum exo somes from sepsis patients injected by tail vein) and control group (normal saline injected by tail vein), Arterial blood gas analysis, serum inflammatory mediators detection, white blood cell count in bronchoalveolar lavage fluid (BALF) , patho logical observation of lung tissue and detection of wet/dry mass (W/D) ratio of lung tissue were performed in rats of each group. Results Compared with sham operated group, the PH value and PaO2 in sepsis group and control group were sig nificantly lower than those in sham operated group ? while PaCC)2 was significantly higher than those in sham operated group. PaC)2 in experimental group was lower than that in sepsis group, while PaCC)2 was higher than that in sepsis group (P<0. 05). Compared with sham operated group, serum levels of TNF a, IL lg, IL 6 and IL 10 in sepsis group and control group were significantly higher than those in sham operated group. The serum levels of TNF a, IL 1 beta, IL 6 and IL 10 in experimental group were higher than those in sepsis group (P<0. 05 ), The number of white blood cells in BALF of sepsis group and control group was significantly higher than that of sham operated group, while the number of white blood cells in BALF of experimental group was higher than that of sepsis group (P<0. 05), The W/D ratio and pathological score of the lung tissue in the sepsis group and the control group were significantly higher than those in the sham group ’ while the W/D ratio and pathological score of the experimental group were higher than those in the sepsisgroup(P<0. 05), Conclusion Exosomes in serum of sepsis patients could further aggravate acute lung injury in septic rats ? which might be related to the aggravation of pulmonary inflammatory response by Exosomes.
