铝碳酸镁联合艾司奥美拉唑对胃溃疡黏膜组织及微血管形态的影响

摘要:
目的 探讨铝碳酸镁联合艾司奥美拉唑对胃溃疡黏膜组织及微血管形态的影响。方法 回顾性分析合肥市第一人民医院2016年1月~2017年1月收治的80例胃溃疡患者临床资料,按治疗方式分为对照组(n=41)和观察组(n=39)。其中,对照组患者接受常规治疗(铝碳酸镁1000mg,3次/d,连续用药15d);观察组在铝碳酸镁治疗的基础上,联合艾司奥美拉唑治疗(艾司奥美拉唑20mg,2次/d,连续用药15d)。比较两组患者临床疗效、微血管形态、炎性因子及胶原纤维沉积面积。结果 两组患者临床疗效比较,差异有统计学意义(P<0.05);观察组患者圆点状、短棒状、无结构比例显著低于对照组(P<0.05),分支状比例显著高于对照组(P<0.05),网络状、绒毛状比例差异不明显(P>0.05)。治疗前,两组患者炎性因子水平比较,差异无统计学意义(P>0.05);治疗后,两组患者10、15d时炎性因子水平低于治疗前,且观察组患者IL-8、IL-10、TNF-α水平显著低于对照组(均 P<0.05)。治疗前两组患者胶原纤维沉积面积比较,差异无统计学意义(P>0.05);治疗后,观察组患者胶原纤维沉积面积显著高于对照组(P<0.05)。结论 铝碳酸镁联合艾司奥美拉唑能显著清除胃溃疡黏膜组织,改善微血管形态,减少炎性反应,可在临床推荐、应用。

Abstract:
Objective To investigate the effect of hydrotalcite combined with esomeprazole on mucosal tissue and microvascular morphology of gastric ulcer. Methods The clinical data of 80 patients with gastric ulcer admitted in the first people′s Hospital of Hefei from January 2016 to January 2017 were analyzed. In the control group, 41 patients received routine therapy (1000mg hydrotalcite, 3 times/d, for 15 days). In the observation group, 39 patients received hydrotalcite combined with esomeprazole (20mg esomeprazole, 2 times/d, for 15d). The clinical efficacy, microvascular morphology, inflammatory factors and collagen deposition area were compared between the two groups. Results The clinical efficacy of the two groups were significantly different (Z=-4.645, P= 0.000). The ratio of round spot pits, short rod pits and no structure in the observation group was lower than that in the control group (P<0.05). While the ratio of branchlike was higher in the observation group than in the control group (P<0.05). No difference was found in the ratio of networklike, villous between groups (P>0.05). The levels of inflammatory rs in the two groups had no significant difference before treatment (P>0.05) and were significantly decreased at the 10d and 15d. The levels of IL-8, IL-10 and TNF-α in the observation group were significantly lower than those in the control group (P<0.05). The collagen deposition area between the two groups before treatment had no significant difference (P>0.05). After treatment, the deposition area of collagen fiber in the observation group was significantly higher than that in the control group (P<0.05). Conclusion The hydrotalcite combined with esomeprazole can significantly clear the mucosal tissue of gastric ulcer, improve microvascular morphology and reduce inflammatory response.
