凝血指标及血清炎性因子在肺癌和肺部感染患者中的相关性

摘要:
目的 探讨凝血指标及血清炎性因子在肺癌和肺部感染患者中的相关性。方法 选取我院2017年7月~2018年10月收治的肺癌及肺部感染患者共90例为研究对象,按疾病分为肺癌非小细胞组、肺癌小细胞组及肺部感染组,每组30例;并选取同期常规健康体检者30例为健康组。检测并比较所有研究对象的凝血指标PT、Fib、D-D、TT及APTT水平;血液学分析指标PDW、MPV、WBC、NEU、Mono及LYM水平;炎性因子IL-6β、IL-1β及IL-8β水平。结果 肺癌非小细胞组与肺部感染组的PT、Fib、D-D、TT与健康组比较,差异有统计学意义(P<0.05);肺癌小细胞组Fib、D-D、TT与健康组比较,差异亦有统计学意义(P<0.05);肺癌非小细胞组与肺癌小细胞组的PT、TT数值明显低于肺部感染组(P<0.05)。与健康组比较,肺癌非小细胞组Mono、NEU显著升高,LYM显著降低;肺癌小细胞组Mono升高,LYM降低;肺部感染组NEU、Mono及WBC值显著升高,PDW、LYM显著降低(均P<0.05)。与健康组比较,肺癌非小细胞组、肺癌小细胞组及肺部感染组的IL-1β、IL-6β及IL-8β水平显著升高;与肺部感染组比较,肺癌非小细胞组、肺癌小细胞组IL-1β、IL-6β及IL-8β水平显著降低(均P<0.05)。结论 肺癌及肺部感染患者均存在炎性反应及高凝状态,且癌症患者的炎性反应低于感染患者,高凝状态高于感染患者。

Abstract:
Objective To study the correlation of coagulation indexes and serum inflammatory factors in patients with lung cancer and pulmonary infection. Methods From July 2017 to October 2018, 90 patients with lung cancer and pulmonary infection were selected as the research objects. The patients were divided into lung cancer non-small cell group, lung cancer small cell group and lung infection group. 30 cases of routine physical examination in the same period were selected as the healthy group. The levels of Pt, FIB, D D D, TT and APTT were measured and compared. The levels of Pt, FIB, D-D, TT and APTT, PDW, MPV, WBC, neu, mono and lym were measured and compared. The levelsof inflammatory factors IL-6β, IL-1β and IL-8β were observed. Results The difference of Pt, FIB, D D D, TT between non-small cell lung cancer group and pulmonary infection group was statistically significant (P<0.05). The difference of FIB, D D D, TT between the small cell lung cancer group and the healthy group was also statistically significant (P<0.05). The values of Pt and TT in non-small cell group and small cell group were significantly lower than those in lung infection group (P<0.05). Compared with the healthy group, MONO and NEU were significantly higher and lym was significantly lower in the non-small cell lung cancer group. In lung cancer small cell group, MONO increased, LYM decreased; in lung infection group, NEU, MONO and WBC increased significantly, PDW and lym decreased significantly (P<0.05). The levels of IL-1β, IL-6β and IL-8β in non-small cell group, small cell group and lung infection group were significantly higher than those in healthy group. The levels of IL-1β, IL-6β and IL-8β in non-small cell lung cancer group and small cell lung cancer group were significantly lower than those in pulmonary infection group (P<0.05). Conclusion The inflammatory response and hypercoagulability of lung cancer and lung infection patients were lower than those of infected patients, and hypercoagulability was higher than those of infected patients.
