肌骨超声对针刀治疗膝关节骨性关节炎的疗效评价

摘要:
目的 探讨肌骨超声(MSUS)评价针刀治疗膝关节骨性关节炎(KOA)的临床应用价值。方法 选取辽宁中医药大学附属医院2017年2月~2018年2月收治的60例KOA患者,均在行针刀治疗当日及治疗5周、6个月时行MSUS检查,观察关节积液厚度、滑膜厚度、滑膜血流信号分级和膝关节软骨分期变化,并记录疼痛视觉模拟评分(VAS)及Lequesne指数评分。结果 治疗后与治疗前相比,患者关节积液厚度、滑膜厚度明显降低(P<0.05),滑膜血流信号分级明显改善(P<0.05),且呈时间依赖性(P<0.05);患者治疗前后膝关节软骨分期无明显变化(P>0.05)。 治疗后与治疗前相比,患者膝关节疼痛VAS评分、Lequesne评分均明显降低(P<0.05),且呈时间依赖性(P<0.05)。 Spearson相关分析显示,治疗前后关节积液厚度、滑膜厚度、血流信号分级等超声指标均与VAS评分、Lequesne评分呈正相关(P<0.05)。结论 MSUS能敏感检测检KOA患者针刀治疗前后关节积液、滑膜厚度、滑膜血流信号的改变,对疗效评价具有重要意义。

Efficacy of musculoskeletal ultrasound and needle-knife in the treatment of knee osteoarthritis

Abstract:
Objective To observe the application value of musculoskeletal ultrasound (MSUS) in the efficacy of knee osteoarthritis (KOA) joint fluid by needleknife treatment. Methods 60 patients with KOA admitted to the hospital from February 2017 to February 2018 were selected and treated with needle-knife for 5 weeks, and they were given MSUS before and after treatment. The joint effusion thickness, synovial thickness, grades of synovial blood flow signal and cartilage stages of knee joint were observed, and pain visual analogue scale (VAS) score and Lequesne index score were recorded. Results Compared with before treatment, the joint effusion thickness and synovial thickness were significantly reduced after treatment (P<0.05), and the grades of synovial blood flow signal were significantly improved (P<0.05), showing time dependence (P<0.05). There were no significant changes in cartilage stages of knee joint before and after treatment P>0.05). Compared with before treatment, the knee pain VAS score and Lequesne score were significantly decreased after treatment (P<0.05), showing time dependence (P<0.05). Spearson correlation analysis showed that the ultrasound indexes of joint effusion thickness, synovial thickness and grades of blood flow signal before and after treatment were positively correlated with VAS score and Lequesne score (P<0.05). Conclusion MSUS examination shows that needleknife treatment can improve joint effusion, synovial thickness and synovial blood flow signal in patients with KOA, which is of great significance for the evaluation of efficacy.
