氟牙症的微创治疗及临床效果评价

摘要:
【摘要】 目的 评估以家庭漂白为主的氟牙症微创治疗的临床效果及不良反应。方法 38例氟牙症患者736颗氟牙症患牙,按临床分型分为轻、中、重3组,联合应用釉质微研磨、10%过氧化脲凝胶家庭漂白及渗透树脂修复的微创治疗方案进行治疗,使用Vita Bleachedguide 3DMASTER 比色板记录治疗前后比色值,评价治疗效果。同时使用VAS评价3组患者治疗不同阶段的牙本质敏感情况,并通过问卷调查患者的满意度。结果 3组氟牙症患牙治疗前后比色值均有显著性差异(P<005)。各组在家庭漂白治疗后的VAS评分与治疗前相比亦有显著性差异(P<005),治疗后1周与治疗前比较,无显著性差异(P>005)。100%的患者认为通过治疗使牙齿颜色得到“显著改善”或“有改善”。 结论 通过联合应用釉质微研磨、家庭漂白及渗透树脂修复的微创治疗氟牙症方法是安全有效的,符合牙科微创治疗的原则,可进一步推广和研究。

Abstract:
【Abstract】 Objective To evaluate the clinical effects and adverse reactions of minimally invasive treatment of dental fluorosis. Methods 38 patients with dental fluorosis, 736 teeth, were divided into three groups according to the Smith classification: mild, moderate, and severe. The combination of enamel microabrasion, 10% carbamide peroxide athome bleaching, and penetrating resin technology were given to each patients. The effect of the treatment was evaluated by Vita shade guide matching. The teeth sensitivity was evaluated by Visual Analogue Scale, and questionnaires were used to survey the satisfaction of patients. Result There was a significant difference in SGU values before and after treatment of the three groups (P<005). The VAS scores of all groups after home bleaching treatment were significantly different from those before treatment (P<005). There was no significant difference of VAS scores between ‘1 week after treatment’ and ‘before treatment’ (P>005). 100% of patients thought that the color of the teeth was "significantly improved" or "improved" by treatment. Conclusion The combination of enamel microabrasion, athome bleaching, and Resin Infiltration is a safe and effective method for minimally invasive treatment of dental fluorosis. It is in line with the principles of minimally invasive dental treatment and is worthy of further promotion and research.
