慢性牙周炎患者种植修复后的临床疗效研究

摘要:
目的 探讨慢性牙周炎对种植修复后临床疗效的影响及其与种植体周围炎症及局部氧化应激的关系。方法 80例牙列缺损并接受义齿种植的患者根据术前是否患有慢性牙周炎分为慢性牙周炎组40例和健康对照组40例,两组患者固定义齿修复完成后第1、3、6、12个月进行复诊,记录1年种植体的存留率。拍摄根尖片检测种植体边缘牙槽骨吸收量(MBL),记录探诊深度(PD)、改良菌斑指数(mPLI)和改良龈沟出血指数(mSBI)。ELISA法检测龈沟液中白介素-1β(IL-1β)、白介素-6(IL-6)、超氧化物歧化酶(SOD)、谷胱甘肽过氧化酶(GP-x)、丙二醛(MDA)水平。结果 两组种植修复后,随着时间的延长,mPLI、mSBI、PD、MBL均呈升高趋势,不同观察时间组间比较差异均有统计学意义(P<0.01)。两组种植修复后,慢性牙周炎组mSBI、PD、MBL值均明显高于健康对照组(P<0.01)。两组种植修复后,随着时间的延长,龈沟液中IL-1β、IL-6、MDA水平均逐渐增加,SOD、GPx水平逐渐降低,组间比较差异有统计学意义(P<0.05或P<0.01)。种植修复后各时间点,慢性牙周炎组龈沟液中IL-1β、IL-6、MDA水平均明显高于健康对照组,SOD、GP-x水平明显低于健康对照组,差异均有统计学意义(P<0.01)。经种植修复1年,各组种植体存留率100%。结论 慢性牙周炎患者接受义齿种植短期内疗效可靠,但出现边缘骨吸收现象较为明显,种植体周围局部微环境存在炎症及氧化应激现象。

The clinical efficacy of implant restoration in patients with chronic periodontitis

Abstract:
Objective To explore the effect of implant restoration in patients with chronic periodontitis, and the relationship with inflammation and oxidative stress. Methods 80 patients with dental defect and received implant restoration were grouped chronic periodontitis group and healthy control group, according to suffering from chronic periodontitis preoperative or not. 1, 3, 6, 12 months after implant restoration, the retention rate of 1 year was recorded. The root tips were detected to recorded the MBL, PD, mPLI and mSBI. ELISA method was used to detect the IL-1β, IL-6, MDA, SOD and GPx in gingival crevicular fluid. Results After implant restoration, the mPLI, mSBI, PD, MBL were all elevated with time (P<0.01), which were higher in chronic periodontitis group than healthy control group (P<0.01). After implant restoration, IL-1β, IL-6 and MDA were all elevated with time (P<0.01), SOD and GP-x were all reduced (P<0.01). After implant restoration, IL-1β, IL-6 and MDA all higher in chronic periodontitis group than healthy control group (P<0.01), SOD and GP-x were all lower than healthy control group (P<0.01). The retention rate of implants in each group was 100% in 1 year.Conclusion The clinical efficacy of implant restoration in patients with chronic periodontitis obviously in the short term. The phenomenon of marginal bone absorption was obvious, and there was inflammation and oxidative stress in the local microenvironment surrounding the implant.
